NY State ID (if you are out of state, you will need to obtain a NY State ID, see process at this link)
Valid Drivers License
Client Conflict of Interest Attestation
Active and valid NY State Medical License (RN, NP, PA License)
Basic Life Support (BLS) Certification
Physical exam signed and dated by a physician within the last year
PPD or QuantiFeron (within 1 year) or chest X-Ray (within 5 years)
TDAP within 10 years
Quantitative Titers for MMR and Varicella
Hepatitis B vaccination schedule or Declination form
Proof of COVID shot
Drug test sent by OrangeTree
Seasonal Flu Vaccination may be required based on State requirements during Flu Season
